A story published in the Oct. 22 edition should have directed those who need a vaccine card replacement through Washington Health System to email their full name, date of birth, and phone number to covidvaccine@whs.org. Instructions will then be provided on how to pick up a replacement card in person. You cannot walk into a physician’s office and get a vaccine card replacement.
